Care Finder and Carers Gateway

Representatives from Northern-Sydney-based Non-Profit and health services provider, Your Side, are pleased to be joining Neutral Bay Community Centre to present a combined talk on two vital services: Care Finders and Carer Gateway.

Both services are provided by Your Side across the Northern Sydney area and are available to support both those who require and those who provide care.

The Care Finder program is a free service that helps people to:

• Set up aged care services

• Change aged care services

• Connect with other supports like housing, health or mental health.

The Carer Gateway program, in partnership with Your Side and the Benevolent Society, is here to support you if you regularly look after someone in your life with a disability, chronic illness, mental health condition, or who is frail aged.  Carers can access dedicated services and support to help you manage daily challenges, improve your health and well-being, and plan for the future.

The cost is $5 and Morning Tea will be served.
